The Asian MSK US meeting 2025, hosted by Dr. Omodani, is the second musculoskeletalultrasound meeting. This year's event was held at the Kongo Noh Theatre, a dedicated Noh theater in Kyoto, making it quite unique.
Besides the host country, Japan, participants included doctors from Taiwan, Hong Kong, South Korea, Thailand, the Philippines, Indonesia, Bangladesh, India, and the United States. Many expert physicians from Taiwan participated in this conference. ultrasound also took this opportunity to meet Dr. Mark Lai and Dr. Peter Siu, MSK ultrasound experts from Hong Kong, and Dr. Panat Nisityotakul, an expert from TAME in Thailand.
In addition to learning from numerous ultrasound experts from Japan and other countries, Dr. Yuji Okuno, the originator of Transarterial Microembolization (TAME), presented his research findings on the treatment of tennis elbow and golf elbow using the ultrasound guided therapy (simplified TAME, Donchu treatment).
Tennis elbow is a tendinopathy characterized by micro-tears in the tendons of the forearm extensor muscles (particularly the extensor carpi radialis brevis, ECRB) at their attachment to the lateral epicondyle of the humerus. These tears fail to heal due to prolonged repetitive use, leading to tendon degeneration. These muscles attach to the lateral epicondyle of the elbow. Repetitive movements or activities, in addition to causing micro-tears and degeneration of the tendons, can also lead to chronic inflammation and pain.

The US-guided simplified TAME (ultrasound), also known as Donchu treatment, does not require the placement of a vascular puncture guide sheath, catheter, or microcatheter to select the vessel, as with the conventional Transarterial Microembolization (TAME). Instead, it involves direct injection through a fine needle puncture into the proximal artery under ultrasound guidance. Therefore, the treatment time is much shorter, and the recovery period and complications after injection are also lower. However, its efficacy is relatively poor. Therefore, after using the [Simplified Transarterial Microembolization (TAME)], it can be combined with local injections/regenerative injection therapy for follow-up enhancement to achieve better therapeutic results.
In conclusion, summarizing the key takeaways from this conference, besides future efforts in [ultrasound diagnostics combined with MRI interpretation], we will further refine and integrate pain management techniques in areas such as [local injection/regenerative injection therapy]; [traditional angiography/ultrasound guided simplified Transarterial Microembolization (TAME)]; [X-ray fluoroscopy guidance/ultrasound guidance/computed tomography (CT) guidance/nerve release/nerve block]; and high-frequency thermocoagulation therapy. We aim to address more intractable [musculoskeletal joint and spinal pain, MSK pain].
