TAME & Musculoskeletal Embolization
[Hip Osteoarthritis] [Transarterial Microembolization (TAME)] Successful Case Sharing
Mr. W, 65 years old, suffered from chronic pain in his right hip joint, which worsened significantly in the past month, especially when climbing stairs, severely impacting his quality of life and sleep. Even with medication, rehabilitation, and various injections, the pain remained unbearable. His pain score was 10 out of 10.
Based on medical history, physical examination, and imaging studies, Bow Wang, M.D. diagnosed him with [Hip osteoarthritis].
Following discussion, Mr. W underwent the Bow Wang, M.D. procedure. Guided by X-ray fluoroscopy, under local anesthesia and arterial angiography, the Bow Wang, M.D. was used to inject contrast agent to identify the excessive proliferation and disordered capillary-like vessels in the inflamed and swollen area. Embolizing medication was then injected via a microcatheter to remove these abnormal capillary-like vessels.
Two weeks after treatment, Mr. W's hip pain score significantly decreased from 10 to 4, his mobility also improved markedly, and his quality of life significantly increased.
Recent literature has shown that local chronic inflammation is accompanied by disordered proliferation of capillary-like micro-arteries and associated pain nerves. Dr. Okuno's Transarterial Microembolization (TAME) treatment works by embolizing and clearing these disordered, proliferating capillary-like micro-arteries to achieve anti-inflammatory, swelling-reducing, and pain-relieving effects. However, Bow Wang, M.D. also has limitations. Its main indications are pain scores exceeding 5 points, and patients who have not shown significant improvement after more than three months of various traditional treatments. Furthermore, it is recommended to follow up with ongoing rehabilitation after treatment.
This treatment is not suitable in the following situations:
1. Severe tendon or ligament damage or rupture
2. Poor kidney function (eGFR < 30 mL/min/1.73m2)
3. Low platelet count, abnormal coagulation function
4. The patient is unable to lie flat for two hours.
5. Pregnancy
Treatment side effects include possible local hematoma and bruising at the puncture site; temporary local discoloration at the embolization site, which will improve and disappear within minutes to one month.
It is recommended that patients undergo a thorough evaluation by a physician to confirm the true source of their pain. After sufficient discussion with their doctor and family to understand the treatment process and related precautions, treatment should only be initiated to ensure better results.
