I am honored to have been invited by the Taiwan Pain Medicine Association and Dr. Pan Chien-Li to have the opportunity to exchange experiences on medical image interpretation and Transarterial Microembolization (TAME) with experts from various fields on December 3, 2022. I am also grateful for the support of my boss, Dr. Wang Chien-Kuo. In the future, I hope to invite experts from various fields to the Radiology Association for an exchange and sharing on pain treatment.

As a radiologist specializing in musculoskeletal, my expertise lies in image interpretation and minimally invasive pain intervention treatment. Since joining the Taiwan Pain Medicine Association, I have been able to learn from many experts in various aspects, including medical history, physical examination, pain mechanisms, biomechanics, and various pain-related treatments.

I was also very happy to participate in the Advanced Knee Pain Intervention Workshop held yesterday, October 8, 2022, hosted by the Taiwan Pain Medicine Association and Chi Mei Rehabilitation Department. I am extremely grateful for the hard work of all the lecturers and staff.

Chronic inflammation leads to microvascular proliferation, inflammatory infiltration, and fibrous scar tissue. Besides the damage inflammatory cells cause to normal tissue, it also disrupts the body's repair mechanisms, further leading to various joint, tendon, and soft tissue diseases.

Treatment for chronic inflammation is divided into "additive" and "subtractive" approaches. For example, anti-inflammatory analgesics (NSAIDs) and steroids belong to the "subtractive" approach, used to stop destructive inflammation. Proliferative therapies, such as glucose solutions, PRP, and PLT injections, belong to the "additive" approach, promoting the restart of previously halted repair mechanisms.

The mechanism of 【Transarterial Microembolization (TAME)】involves blocking disordered proliferation of microvessels to achieve (a) accompanying pain nerve apoptosis. (b) inhibiting and blocking the infiltration and apoptosis of destructive inflammatory cells.

Example: (a) Disordered proliferation of micro-arteries in the frozen shoulder joint capsule (b) After the micro-arteries were cleared by embolization, the patient's pain and range of motion were greatly improved.
