Mr. K, 51, has been a saxophone player for many years, enjoying it daily. However, frequent playing has caused him to experience pain in both elbows for over three years.

A doctor's examination diagnosed him with bilateral lateral epicondylitis, commonly known as tennis elbow.

Mr. K had tried patches, local injections including steroids, high-concentration glucose solution, and PRP, and underwent rehabilitation treatments, but the pain persisted, and his pain index reached 6-7. After various treatments proved ineffective, he went to the Interventional Radiology Clinic of National Cheng Kung University Hospital for further evaluation and interventional treatment.

Through imaging evaluation, Department of Medical Imaging, National Cheng Kung University HospitalBow Wang, M.D. found damage to the common extensor tendon of the patient's wrist extensor muscles at the junction of the muscle and tendon. Arterial angiography revealed enlarged capillary-like vessels, which were then embolized.

Two weeks after embolization, the patient's pain improved by 30%.

However, Bow Wang, M.D. reminds users that transarterial embolization (TACE) for "Lateral epicondylitis of the humerus" also has limitations. Its main indication is a pain index exceeding 5 points, and patients who have not shown significant improvement after more than three months of various treatments.

This treatment is not suitable in the following situations:

1. Severe tendon or ligament damage or rupture
2. Pain accompanied by other nerve compression symptoms, such as numbness
3. Poor kidney function (eGFR < 30 mL/min/1.73m2)
4. Low platelet count, abnormal coagulation function
5. The patient is unable to lie flat for two hours
6. Pregnancy

Side effects of the treatment include local hematoma and bruising at the puncture site; and temporary local discoloration at the embolization site, which will improve and disappear within minutes to a month.

Bow Wang, M.D. points out that transarterial embolization (TACE) for pain relief requires evaluation by a professional physician before it can be performed. Its main effect is pain relief, but it cannot reverse tears in already degenerated joints and tendons. Regular care and adequate rest for the joints and tendons are far more important than treating excessive strain!
